Arrhythmias and treatments Flashcards
What does paroxysmal mean?
Starts and ends abruptly
opposite of sustained
What are the 5 classes of anti-arrhythmic drugs?
1- Sodium blockers 2 - beta blockers 3 - potassium blockers 4 - calcium blockers 5 - miscellaneous
What is the vagal manoeuvre?
Carotid sinus massage or gentle eyeball pressure
Try if supraventricular tachycardias
How can supraventricular tachycardias be treated?
IV esmolol or diltiazem
(or oral)
Vagal manoeuvre?
What is the 1st line drug for atrial fibrillation?
Digoxin
After digoxin, what other drugs can be used to treat atrial fibrillation?
Diltiazem
+/- Beta blockers - NEVER IN UNCONTROLLED CHF (worsens poor systolic function)
At what heart rate does ventricular tachycardia require treatment?
> 200bpm
How is ventricular tachycardia treated with emergency IV treatments?
Lidocaine
Esmolol
Amiodraone CRI
What oral anti-arrhythmics can be used to treat ventricular tachycardia?
Sotalol
Magnesium chloride
(Amiodarone - hepatotoxic long term)
What causes atrial standstill (type of bradycardia)
Hyperkalaemia
What breed of dog and what endocrine condition are prediposed to atrial standstill?
WHWT
Addison’s - hyperkalaemia
(Consider urinary causes of hyper K)
How can you test to see if bradyarrhythmias are vagally mediated?
Atropine response test
If >50% increase in HR, vagal mediated
What is the life-threatening treatment for bradyarrhythmias?
Terbutaline - less risk of arrhythmias
Atropine?
What oral medications can be used to treat bradyarrythmias?
Terubtaline
Theophylline
Propanetheline
