O- Long Head: Supraglenoid Tubercle
Short Head: Coracoid Process
I- Radial tuberosity, bicipitial aponeurosis
A- Flexing the shoulder, flexion of elbow and supination of the wrist

Bracialis
O- Anterior distal 2/3 of humerus
I- Coronoid process of the ulna
A- Flexion of the elbow
N- Musculocutaneous Nerve
Triceps Brachii
O- Long Head: Infraglenoid tubercle of scapula
Lateral Head: Proximal 1/3 of the humerus lateral to radial groove
Medial Head: Distal 1/3 of the humerus medial to lateral groove
I- Olecranon Process
A- Extension of the humerus, extension of the elbow
N- Radial Nerve
Brachioradialis
O- Supracondylar line of the humerus
I- Radial Styloid
A- Flexion of the elbow (mid-prone)
N- Radial Nerve
Anconeus
O- Posterior aspect of lateral epicondyle of the humerus
I- Lateral surface of the olecranon process
A- Extends the elbow
N- Radial
Pronator Teres
O- Medial epicondyle of the humerus
I- Anterior surface of radius (1/2 way)
A- Pronation of the forearm
N- Median Nerve
Pronator Quadratus
O- Medial anterior surface of the distal ulna
I- Lateral anterior surface of the distal radius
A- Pronation of the forearm
N- Median Nerve
Supinator
O- Lateral epicondyle, supinator crest of ulna, radial collateral ligament, annular ligament
I- Lateral, posterior and anterior surface of radius
A- Supination of the forearm

Extensor Carpi Radialis Longus
O- Lateral Epicondyle
I- Base of the 2nd metacarpal
A- Extension of the wrist, abduction of the wrist

O- Medial epicondyle, medial margin of the olecranon process of the ulna
I- Pisiform, hook of hamate and base of 5th metacarpal
A- Flexion of the wrist, adduction of the wrist
N- Ulnar nerve
Palmaris Longus (if show)
O- Medial Epicondyle
I- Palmar Fascia
A- Draws tension on the palmar fascia
N- Median Nerve
