step wise approach
step 1: education of GAD and active monitoring
step 2: low intensity psychological intervention (i.e. self help groups)
step 3: high intensity psychological intervention (i.e. CBT, relaxation) or medications
step 4: high specialised input
1st line drug treatment for GAD ?
sertraline
if ineffective offer alternative SSRI or SNRI i.e. duloxetine, venlafaxine
if patient cannot tolerate SSRI or SNRI what is 3rd line?
pregabalin
ICD-10 classification of generalised anxiety disorder ?
period of 6 months with prominent worry, tension and feelings of apprehension about every day events and problems
+ at least 4 symptoms i.e. palpitations, breathlessness, chest pain, dizziness etc
