low weight (85% expected or 1/3 percentile) lack of weight gain significant oedema physiological decompensation - electrolyte disturbance - cardiac disturbance - altered mental status - orthostatic difference > 30 temp < 36 pulse < 45 psychosis or high risk suicide symptoms unmanageable by outpatient
1st line management options in treatment of anorexia in adults ?
one of the following;
- Maudsley anorexia nervosa treatment in adults (MANTRA)
- CBT
- specialist supportive clinical management (SSCM)
1st line treatment for management of anorexia in children/adolescents ?
anorexia focussed family therapy
ICD-10 criteria for anorexia nervosa ?
restriction of food intake
compensatory behaviours when food cannot be avoided
fear of gaining weight
BMI < 18 OR < 15% percentile
in post menarche women, amenorrhoea for 3 months
ICD-10 criteria for bulimia ?
episodes of binge eating with feeling of lack of control
followed by compensatory behaviour (e.g. purging or non-purging)
binging and purging must occur at least 2x per week for 3 months
dissatisfaction with body weight and shape
treatment options for bulimia?
bulimia nervosa focussed guided self-help
eating disorder focussed CBT
in children - bulimia focussed family therapy
