Antiplatelet drugs Flashcards
What antiplatelets work by inhibition of platelet ADP (P2Y12) receptor? Which ones are irreversible and which one is reversible?
Clopidogrel and prasurgrel are irreversible
Ticagrelor is reversible binding
What antiplatelet works by inhibition of phosphodiesterase in platelets? this inhibiton increases cAMP which increases PGI2 anti-aggregation effect
Dipyridamole
What antiplatelets block platelet GIIb/IIIa receptor preventing fibrinogen binding and platelet aggregation?
- Abciximab- monoclonal antibody
- Eptifibatide- cyclic peptide
- Tirofiban- non-peptide
How often is Clopidogrel and prasurgrel taken?
once daily
how often is ticagrelor taken?
PO BID
- how are Abciximab, Eptifibatide, Tirofiban administered?
parenteral
How is aspirin eliminiated?
hepatic
how is clopidogrel eliminated?
well its activated by CYP2C19 then renal and fecal elimination
How is Prasugrel eliminated?
Well its activated by CYP3A4/2B6 thats all we know
How is Ticagrelor eliminated?
CYP3A4 metabolism to active metabolite
how are Abciximab, Eptifibatide, Tirofiban eliminated and list them in order of least half life time to greatest.
- renal elimination
2. A
how is dipyridamole eliminated?
hepatic: Phase II conjugation
Adverse effect of aspirin?
- gastric upset
2. bleeding
Adverse effect of clopidogrel, ticagrelor, prasurgel.
- Bleeding ([P]-[T]>[C])
- clopidogrel causes dyspepsia, gastritis and headache
- ticagrelor causes dyspnea, bradyarrhythmias
what adverse effect dose Abciximab, Eptifibatide, Tirofiban have
Bleeding
