-maternal afp ; High
-family HX, Spina Bifida
-blood group, indirect coombs

-it is a developmental defect of the brain which occurs between 5 and 8 weeks.
-the condition is always fatal to baby after birth.
-SO two option terminate now or ct until labour.

1.TERMINATION NOW
i. Use of prostaglandins
-may take several hours, or days
-uterine contractions similar to labour
-possible need for curretage, to remove placental
ii.sx dilatation and curretage
-risk of cervical incompetence
-procedure difficult except in expert hands
2.POSTMORTEM EXAM
-to check other abnormaities, which may influence advice of further pregnancies
3.RECURRENCE
-3-5percent NTD
-FA 5mg day prior to conception up to 12 weeks gestation
-maternal AFP at 16weeks
