Pregnant women have a mean Hb of 110 1st trimester
<110 in 1st term
<105 in 2nd term
<100 post partum
<70 URGENT REFER
Types of Aneamia in pregnancy and RF
Many ypes - main tend to be
Iron def aneamia -(blood loss, red intake, inc use)
Folate -> care also with neural tube defects
B12- neural tubes
RF- Mutlple pregnancies Lots of vomit in morn aneamia before preg 2 preg together pregnant teen diet (vegan etc)
Sx and ix of Aneamia in pregnancy
lethargy, weakness, dizzinesss, SOB, trouble focus, tachycardia/tachypnea, pale skin/lips/eyes
B12 specific- glossitis, depression, psychosis, parasthesia, neuropathy
Ix - screen at booking and 28w
FBC
heamtinics
Blood film
Mx of Aneamia in pregnancy
Supplements -
if ferritin okay OTC
if not 100-200mg OD iron - rechech 2-3weeks
ferrous sulphate, folic acid, B12
(always care to not give folic if b12 could be cause)
Increase diet intake
iron-animal, grean leaft veggie
B12-meat and dairy
folate- green leaft veggy, nuts, yeast
Intra-partum - deliver in consutlant lead unit IV acess on admision active mx of 3rd stage active of PPH consdier prophylatic syntocion infusion
Complications of Aneamia in pregnancy
Preterm or LBW baby
postpartum depression
Child with development delay
Spina bifida
but prognosis is overall v good
