Mechanism of Action
Bactericidal
Most often used in combination with other antibiotics such as β-Lactams (synergistic effect)
Indications
Treatment of serious gram- infections
Gram+ cocci only when COMBINED with penicillin
Prophylaxis - GI/GU tract procedures
Contraindications
Known drug allergy
Can cause fetal harm in pregnant women
Adverse Effects
Can cause serious toxicities, particularly in the kidneys and ears
Ears - hearing loss, tinnitus, dizziness, vertigo
Kidneys - reduced kidney function
Interactions
Risk for nephrotoxicity can be increased with concurrent use of other nephrotoxic drugs
Use with loop diuretics can increase the risk for ototoxicity
Can potentiate anticoagulants such as Warfarin
Neuromuscular blockers
Nursing Considerations/Patient Teaching
Do NOT give to pregnant/lactating women and neonates
Encourage fluid intake
