An inherited disorder affecting the liver and lungs
What is serpinopathy?
Another word for A1AT deficiency as A1AT is a serine protease inhibitor
What does A1AT cause?
Emphysema and cirrhosis
Are children or adults more likely to get liver disease?
Children

Adults
What causes emphysema?
Tissue damage by neutrophil elastase
How might a patient with A1AT deficiency present?
Dyspnoea and other emphysema symptoms, cirrhosis, cholestatic jaundice if child
What blood test would you do?
Serum A1AT levels
What would lung function tests show?
Reduced FEV1 with obstructive pattern
What liver tests would you do?
Liver function tests
What imaging would you do?
CXR or chest CT
What are differential diagnoses?
Asthma, COPD
What general management would you start?
Smoking cessation, vaccination for lung infections and hepatitis
What options are there for lung manifestations?
Treat emphysema normally, A1AT augmentation, lung transplant
What options are there for liver manifestations?
Treat liver disease normally, alcohol avoidance, liver transplant
