Q

What are the uses of Alfentanil?

A
  • rapid onset makes it ideal for analgesia when noxious stimulation is acute, but transient, as associated with laryngoscopy, tracheal intubation, and performance of a retrobulbar block
  • analgesic adjunct in the maintenance of anesthesia with barbiturates or N2O or as primary anesthetic agent for the unduction of anesthesia

Q

What is the MOA of Alfentanil?

A

mu-opioid receptor agonist
mu, kappa, delta
-drug binds to stereospecific receptors at many sites within the CNS, increases the pain threshold, alters pain perception and inhibits ascending pain pathways

Q

What is the Vd of Alfentanil?

A

4 - 6 times smaller than Fentanyl. This is due to lesser lipid solubility and higher protein binding

0.46 L/kg

16
Q

What type of drugs may increase levels of sufentanil?

A

CYP3A4

17
Q

What is the analgesia dose of Alfentanil?

A

5 - 30 mcg/ kg (followed by infusion of 0.4 - 1.7 mcg/kg/min)

18
Q

What is the induction dose of Alfentanil?

A

50 - 200 mcg/kg

19
Q

What is Alfentanil an analogue of?

A

Fentanyl

20
Q

____% exists in an unionized state= ?

A

90%= VERY rapid onset (1.4 min compared to Fentanyl, 6.8 min and sufentanil 6.2 min)

21
Q

Is alfentanil highly protein bound?

A

YES, but highly diffusable

22
Q

Is alfentanil’s diffusion fraction higher, lower, or the same as fentanyl? Why is this weird?

A

HIGHER diffusible fraction than Fentanyl despite more intense protein binding

23
Q

____ as potent as Fentanyl? _____-___ x more potent than morphine

A

1/5th; 10 -20

24
Q

What is the clinical significance of Alfentanil?

A
  • rapid onset is useful for blunting hemodynamic response to noxious stimuli
  • rapid effect due to 90% of drug in non-ionized form
  • used as infusion for outpatient surgery
25
Q

What is the dosage of Alfentanil?

A

10 -20 mcg/kg

26
Q

What isthe infusion rate of Alfentanil?

A

0.25 - 0.75 mcg/kg/min

27
Q

What is the loading dose of Alfentanil?

A

25 - 100mcg/kg

28
Q

What is the maintenance infusion rate of Alfentanil?

A

0.5 - 2 mcg/kg/min

29
Q

What are the additional bolus doses of Alfentanil?

A

5 - 10 mcg/kg
