What is anticoag for in ACS?
Recommended in addition to antiplatelet therapy to improve vessel patency and prevent re-occlusion
What is the MOA of UFH?
Anti-Xa and anti-2a activity
What is the problem w UFH and LMWH?
It can cause HIT, which is a drop in platelet count and increased thrombosis
How is HIT caused?
Caused by formation of antibodies that activate platelets
What screening tests are available for HIT?
ELISA and SRA
What should be done if pt tests positive for HIT?
If pt is + for HIT, stop UFH/LMWH, and tx for HIT while doing SRA for true + HIT diagnosis
How is UFH administered?
As a continuous infusion
What is dosing of UFH based on?
aPTT or ACT
What LMWH drug is used?
Enoxaparin
What is the MOA of LMWH?
Anti-Xa and anti-2a activity
How is LMWH eliminated?
By kidneys
- accumulates in renal impairment
What drug class is bivalirudin?
DTI
What other drug class is bivalirudin not used together with?
Not used together w GP2b/3a inhibitors (except bail out)
What drug class is fondaparinux?
Factor Xa inhibitor
When is fondaparinux used?
- Not commonly used
- Can be used in pts w hx of HIT
