What temp etc would be considered pyrexia of unknown origin?
> 38.3 degrees
persisting for 3 weeks with no diagnosis despite treatment for 1 week
What do you do if a pt has febrile neutropenia?
MED EMERGENCY:
Take 2 culture samples then treat with Abx asap
Look for conditions that need neutrophils e.g. fungal/bacterial
If pt has PUO and is HIV positive- what are you thinking for the differentials?

Kaposi’s sarcoma

What causes typhoid?
Samonella Typhi and Paratyphi
What are the key symptoms of typhoid?
Fever, abdo pain, diarrhoea
Headache
Hepatomegaly
ROSE SPOTS on abdo (in 30%)
What is the gram stain of typhoid?
Gram neg bacilli
Management of Typhoid?
Notifiable
Supportive (IV fluids)
Oral.IV Abx: IV Ceftriaxone

IV Doxycycline

Plasmodium Falciparium

In which organism’s infection would you found Schnuffner’s dots?
Plasmodium Ovale and Plasmodium Vivax
Describe the cell findings in P. Falciparum?
Ring forms (trophozoites) in RBCs and Banana/cresent gametocytes
How do you treat the differnt types of Malaria?
Falciparum: 600 mg Quinine 1 week then doxycycline/clindamycin
Vivax and Oval: Chloraquine then Primaquine
When can you not have Primaquine?
Pregnancy
How do you investigate for P.Falciparum malaria?
Thick blood film: will show parasites
Then thin blood film: will show type of malaria (in Falciparum will see ring trophzoites and Banana/cresent gametocytes)
S/E of quinine?
Arrhythmias
Treatment of mild/mod Falciparum?
PO 600mg Quinine TDS
Then Doxy and Clinda
Treatment of severe Falciparum?
MEDICAL EMERGENCY
IV Artesenate AND quin/doxy/clin therapy
Common symptoms of mild/mod malaria?
Fever
Rigors
Splenomegaly
Headache
Symptoms of severe P.Falciparum malaria in adults?
AAB CRASH
ARDS
Acidosis
Bleeding
Consciousness impaired Renal impairment Anaemia Shock Hypoglycaemia
