The EMS crew shall draw 1 purple tube and 1 red tube for the “source” and label with source name and date of birth. The medic shall also label the date and time, time drawn and place initials on the tubes.

Complete Form 207 for Department of Health

The employee should complete Part I of Form 207 (lines 1-13).The Shift Commander or Training Officer should complete lines 14 – 19.

Before leaving the ER, ensure that a licensed health care professional (MD, DO, RN, PA) complete lines 20, 21, and 22 of OSDH Form 207 and you have copy of everything.

Upon returning from the Emergency Room, fill out accident/illness report and the Employers’ First Notice of Injury Report.

After completing paperwork, the yellow copy should be mailed to the OSDH using the included gray envelope. It will be the responsibility of the Training Officer and/or Shift Commander to ensure this is accomplished.

If washing more than one garment at a time you should use a tagging system for ease of matching the correct shell to the correct liner. Only wash 4 pieces per load.

Best to wash bagged DRD with outer shell components, not liners.

Heavily soiled or spotted areas shall be pretreated.
DO_NOT use Chlorine bleach, chlorinated solvents, active ingredient cleaning solvents.

Elements shall be dried in a well-ventilated area or in a dryer on a “low” or “no heat” setting. It is important that the basket temperature does not rise above 105°F.

For decontamination of your PPE from harmful exposure use the Wash Program 3 for two complete cycles.
