Commanders requesting the issuance of the supplemental firearm shall forward a:
Memorandum, through channels, to the appropriate commissioner.

What is the definition of a special purpose firearm?
Any firearm other than in approved departmentally issued firearm, which is carried on duty by uniformed employees during emergency operations.
Special purpose firearms may only be carried with the written approval of the appropriate:
Division chief. A copy of the approval shall be sent to the appropriate Assistant Commissioner.

Before separation, was regularly employed as a law enforcement officer for an aggregate of ten years or more; or separated from service after completing probation with the Department due to a service-connected disability.
How long is a HR 218 Certification good for?
It is valid for a 12-month period.

(1) Immediately report the incident according to General Order 100.80.
(2) Uniformed employees shall immediately notify their next level of command.

Uniformed employees who have been assigned to office duty because of their involvement in an actual shooting incident where shots were fired at another person whether or not an injury or death resulted shall:
fire a qualifying score on the Task Oriented Qualification Course -Pistol prior to returning to full duty.
All uniformed personnel shall train with the primary firearm on a quarterly basis, shooting a minimum of:
100 rounds. Missed quarterly shoots may be satisfied by completing a make up shoot.
All uniformed employees are required to qualify annually using the:
50-round TOQC -Pistol Course
