in the employee’s Field Folder.

Forward within 5 days of injury.
What must a employee prepare if there is a status change to a first-aid injury or illness?
A CHP 51, Memorandum, with any medical documentation attached. Must be within 24 hours of the change.
When is SCIF notified for a Status Change to First-Aid Injury or Illness?
When medical expenses are incurred. Remove original CHP 121 from Field Folder and forward it with a copy of the CHP 51 to SCIF within 5 days of the change.
What must a supervisor prepare for a Status Change to a Nondisabling Injury or Illness?
Initiate SCIF 3301 and provide to employee within 24 hours and prepare a Status Change Comm-Net message.
A Comm-Net Message shall be transmitted to Disability and Retirement Unit (DRU) within 24 hours of notification of a:
nondisabling (other than record only or first aid) or disabling occupational injury or illness.
The commander or designee shall review the commander’s memorandum and attachment with the employee within:
24 hours of notification of an injury or illness
other than record only or first aid.
