Immune thrombocytopenic Purpura

A

Define
Immunologic destruction of platelets
Normal or increased megakaryocytes in bone marrow

Pathogenesis

Acute

Self limit
Children
Recovery from viral diseases 
Immune complexes containing viral antigens
Formation of ab. Against antigens
Cross react with platelets

Chronic

Adult women child bearing age
Antiplatelet autoantibodies
Ig g in spleen

Clinical

Petichial haemorrhages
Easy bruising
Mucosal bleeding nasal gums hematuria menorrhagia

Chronic spleenomegaly hepatomegaly

Laboratory

Platelets 10k to 50k
Blood film occasional platelets
Marrow increase megakaryocytes
Antiplatelet ig g ab on platelet surface
Platelet survival less than hour

Treatment

Recover spontaneously
Corticosteroid
Immunosuppressive drugs
Splenectomy
