6. Diverticuli of the oesophagus, Achalasia Flashcards
1
Q
Diverticuli types
A
- Zenkers (upper 1/3)
- Traction (mid 1/3)
- Epiphrenal (lower 1/3)
2
Q
Diverticuli basics
A
Outpouching of mucosa
- true: all layers of wall
- false: mucosa only
3
Q
Zenker diverticulum
A
- Only two muscle layers
- Signs: regurgitation, aspiration pneumonia, cough
- Dg: anamnesis, clinical signs, X-ray, endoscopy
- Therapy: diverticulectomy + cricopharyngeal myotomy
4
Q
Traction diverticulum
A
- Lymphatic inflammation
- Mainly symptom free
- Surgery exceptional
5
Q
Epiphrenal diverticulum
A
- Signs: dysphagia retrosternal pain
- Dg: X-ray, endoscopy
- Therapy: only if complains - surgery
6
Q
Achalasia definition
A
failure of the lower esophageal sphincter (LES) to relax that is caused by the degeneration of inhibitory neurons within the esophageal wall
7
Q
Symptoms of achalasia
A
- Progressive dysphagia or paradoxical dysphagia
- Regurgitation of undigested food/saliva
- Weight loss
8
Q
Diagnosis of achalasia
A
- Esophageal barium swallow
- Upper endoscopy
- Esophageal manometry
- Chest X-ray -> shows mediastinum
9
Q
Treatment of achalasia (low surgical risk)
A
- Pneumatic balloon dilatation (2x - 3x)
- Oesophago-cardio-myotomy (Heller), Cardiomyotomy
10
Q
Treatment of achalasia (high surgical risk)
A
- Botulinum toxin injected in the LES
- Nitrates or Ca++ channel blockers
