obese adolescent w/ new onset, sometimes gradual, hip (sometimes knee) pain
SCFE
painless limp in 6-7 y/o boy (hip)
legg-calve-perthes disease (avascular necrosis of the femoral head)
kid w/ focal bone tenderness + fever
osteomyelitis
transient painful limp in 2-6 y/o, not osteomyelitis or septic joint
transient synovitis
intermittent swelling + painful knee, uveitis
JIA
fever followed by desquamation of skin
staph scalded skin syndrome
herald patch followed by diffuse rash of raised lesions, pink/brown color, some w/ scale; “christmas tree rash”
pityriasis rosea
child w/ purpura, colicky abd pain, RBCs + protein in urine
HSP (IgA-mediated vasculitis)
tx for potty-trained child w/ leakage of stool + constipation
encopresis - tx w/ disimpaction, 3-6 mo of stool softener
child w/ bow legs + prominent step abnormality of medial proximal tibial epiphysis
blount dz - bracing up to age 3, surgery after
poisoning: agitation, HTN, cherry-red skin, diaphoresis
cyanide - blood gas shows severe metabolic acidosis w/ high venous O2 levels
tx for child w/ scabies
permethrin cream
sepsis in a newborn w/ bilateral lung infiltrates - mom w/ limited prenatal care
GBS pneumonia - tx w/ ampicillin
2 y/o w/ fever + tender hip joint, widened joint space on XR
septic arthritis - usually s aureus in kids, n gonorrhea in teens
tx for head lice
topical ivermectin in kids >6mo
