Who is responsible for issuing employee ID cards
Employee Services Division
An employee is still considered Honorably Discharged as long as:
They corporate with an investigation and the cite is Category D or less
If an employee wishes to maintain their badge and hat shield
Submit a written request through chain of command
Up to assist chief level
How many years must a civilian do to receive retirement?
10 years with the city. 5 years with HPD IMMEDIATELY PROCEEDING RETIREMENT
When is employee able to keep hat shield and badge while suspended?
15 days or less
Handled at division level
When is an employee unable to keep badge and hat shield?
16 days or more
Handled at IAD level
When an employee’s badge, hat shield, or ID is stolen
The supervisor must give 48 hours
*may be handled as an expedited investigation *
Where does an employee bring the Sig event report, incident report, admin letter with IRF number when they lost badge, hat shield but not the ID

The ID IS REPLACED AT ESD
Who issues the loaner set? Hat shield and badge

For replacement of ID :
Bring copy of report, copy of admin letter signed by your supervisor, and a receipt for restitution from the Office of Budget and Finance to the Employee Services Division
Supervisor’s responsibility when an employee looses hat shield badge or ID?
Investigate the incident and submit to the Division commander a written report that includes:
G) cope of incident report
H) copy of the significant event report
I) employee’s admin letter
J) a seafood the item including its replacement or repair value as determined by ESD for identification card and/or the TAP unit for the HPD badge and/or HPD hat shield shall be added to the expedited form, as applicable
