Tave in 1 or more PCS loops not w/in limits, then be in Mode 2 with Keff < 1.0 w/in 30 minutes

LCO 3.4.3 requirement
PCS pressure, temperature, and heatu/cooldown rates shall be maintained w/in the limits of Figure 3.4.3-1 and 3.4.3-2
LCO 3.4.3 applicability
At all times
LCO 3.4.3 required actions < 1 hour
- Requirements of LCO not met in Modes 1-4, then restore parameter(s) to w/in limits in 30 minutes
- Requirements of LCO not met any time in other than Modes 1-4, then initiate action to restore parameter(s) to w/in limits immediately

PCS Loops - Modes 1 and 2
LCO 3.4.4 requirement
2 PCS loops shall be operable and in operation

LCO 3.4.5 title
PCS Loops - Mode 3
LCO 3.4.5 requirement
- 2 PCS loops shall be operable and 1 PCS loop shall be in operation
- All PCPs may not be in operation for <= 1 hour per 8 hour period provided no operations are permitted that would cause reduction of PCS boron concentration AND core outlet temperature is maintained at least 10 F below saturation temperature
- Forced Circ (starting the 1st PCP) shall not be initiated unless PCS Tc is > 430 F, S/G secondary temperature is <= reactor inlet temperature (Tc), S/G secondary temperature is < 100 F above Tc and SDC is isolated from the PCS and PCS heatup/cooldown rate is <= 10 F/hour, OR S/G secondary temperature is < 100 F above Tc and SDC is isolated from the PCS and PZR level is <= 57%
LCO 3.4.5 applicability
Mode 3
LCO 3.4.5 required actions < 1 hour
No PCS loop operable OR no PCS loop in operation, then suspend all operations involving a reduction of PCS boron concentration AND initiate action to restore 1 PCS loop to operable and in operation immediately
LCO 3.4.6 title
PCS Loops - Mode 4
LCO 3.4.6 requirement
- 2 loops or trains consisting of any combo of PCS loops and SDC trains shall be operable, and either 1 PCS loop shall be in operation or 1 SDC train shall be in operation with >= 2810 gpm flow through the reactor core
- All PCPs may not be in operation for <= 1 hour per 8 hour period provided no operations are permitted that would cause reduction of PCS boron concentration AND core outlet temperature is maintained at least 10 F below saturation temperature

- 1 PCS loop inoperable AND 2 SDC trains inoperable, then initiate action to restore a 2nd PCS loop or 1 SDC train to operable immediately
- No PCS loops or SDC trains operable OR no PCS loop in operation w/SDC flow through the reactor core not w/in limits, then suspend all operations involving reduction of PCS boron concentration AND initiate action to restore 1 PCS loop to operable and in operation OR initiate action to restore 1 SDC train to operable and >= 2810 gpm flow through the reactor core immediately
LCO 3.4.7 title
PCS Loops - Mode 5, Loops Filled
LCO 3.4.7 requirement
- 1 SDC train shall be operable and in operation w/>= 2810 gpm flow through the reactor core and either 1 additional SDC train shall be operable, or secondary side water level of each S/G shall be >= -84%
- All PCPs may not be in operation for <= 1 hour per 8 hour period provided no operations are permitted that would cause reduction of PCS boron concentration AND core outlet temperature is maintained at least 10 F below saturation temperature
- Both SDC trains may be inoperable for up to 2 hours for surveillance testing or maintenance, provided 1 SDC train is providing the required flow through the core, core outlet temperature is maintained at least 10 F below saturation temperature, AND each S/G secondary side water level is >= 84%
- Forced Circ (starting the 1st PCP) shall not be initiated unless S/G secondary temperature is <= reactor inlet temperature (Tc), S/G secondary temperature is < 100 F above Tc and SDC is isolated from the PCS and PCS heatup/cooldown rate is <= 10 F/hour, OR S/G secondary temperature is < 100 F above Tc and SDC is isolated from the PCS and PZR level is <= 57%
- PCPs P-50A and P-50B shall not be operated simultaneously
- All SDC trains may not be in operation during planned heatup to Mode 4 when at least 1 PCS loop is in operation
LCO 3.4.7 applicability
Mode 5 with loops filled
LCO 3.4.7 required actions < 1 hour
1 SDC train inoperable AND any S/G with secondary side water level not w/in limit, then initiate action to restore a second SDC train to operable OR initiate action to restore S/G secondary side water levels to w/in limits immediately
-2 SDC trains inoperable OR SDC flow through the core not w/in limits, then suspend all operations involving a reduction in PCS boron concentration AND initiate action to restore 1 SDC train to operable and in operation with >= 2810 gpm flow through the core
LCO 3.4.8 title
PCS Loops - Mode 5, Loops Not Filled
LCO 3.4.8 requirement
- 2 SDC trains shall be operable and either 1 SDC train in operation with >= 2810 gpm through the core OR 1 SDC train in operation with >= 650 gpm through the core w/2 of 3 charging pumps incapable of reducing boron concentration in the PCS below minimum value necessary to maintain the required SDM
- All SDC pumps may not be in operation for <= 1 hour provided no operations are permitted that would cause a reduction of the PCS boron concentration, core outlet temperature maintained > 10 F below saturation temperature, AND no draining operations to further reduce PCS water volume are permitted
- 1 SDC train may be inoperable for <= 2 hours for surveillance testing provided the other SDC train is operable and in operation
LCO 3.4.8 applicability
Mode 5 with loops not filled
LCO 3.4.8 required actions < 1 hour
- 1 SDC train inoperable, then initiate actions to restore SDC train to operable immediately
- 2 SDC trains inoperable OR SDC flow through the core not w/in limits, then suspend all operations involving reduction of PCS boron concentration, AND initiate action to restore 1 SDC train to operable and in operation with SDC flow through the core w/in limit
LCO 3.4.9 title
Pressurizer
LCO 3.4.9 requirement
The PZR shall be operable with:
- level < 62.8% (N/A in Mode 3 until after a bubble has been established and water level has been lowered to w/in its normal operating band)
- > = 375 kW of heater capacity available from D bus
- > = 375 kW of heater capacity available from E bus with the capability of being powered from an emergency power supply

PZR Safety Valves
LCO 3.4.10 requirement
3 PZR safety valves shall be operable w/lift settings as specified in Table 3.4.10-1 (2580 psia for RV-1039, 2540 for 1040, and 2500 for 1041)
LCO 3.4.10 applicability
- Modes 1 and 2
- Mode 3 with all PCS cold leg temperatures >= 430 F
LCO 3.4.10 required actions < 1 hour
1 PZR safety valve inoperable, then restore valve to operable w/in 15 minutes
LCO 3.4.11 title
PZR Power Operated Relief Valves
LCO 3.4.11 requirement
- Each PORV and associated block valve shall be operable
- Separate Condition entry is allowed for each PORV
LCO 3.4.11 required actions < 1 hour
- 1 PORV inoperable, then close associated block valve w/in 1 hour
- 1 block valve inoperable, then place associated PORV in manual control w/in 1 hour
- 2 PORVs inoperable, then close associated block valves w/in 1 hour
- 2 block valves inoperable, then place associated PORVs in manual control w/in 1 hour
LCO 3.4.12 title
Low Temperature Overpressure Protection System
LCO 3.4.12 requirement
-An LTOP System shall be operable with:
1. both HPSI pumps incapable of injecting into the PCS (only required when any PCS cold leg temperature < 300 F. also doesn’t prohibit the use of HPSI pumps for emergency addition of makeup to the PCS)
AND
2. either 2 PORVs with lift settings as specified in Figure 3.4.12-1 OR PCS is depressurized and a PCS vent capable of relieving >= 167 gpm at 315 psia
LCO 3.4.12 applicability
- Mode 3 when any PCS cold leg temperature < 430 F
- Modes 4 and 5
- Mode 6 when the reactor vessel head is on
LCO 3.4.12 required actions < 1 hour
1 or 2 HPSI pumps capable of injecting into the PCS, then initiate action to verify no HPSI pump is capable of injecting into the PCS immediately
LCO 3.4.13 title
PCS Operational Leakage
LCO 3.4.13 requirement
PCS operational leakage shall be limited to:
- no pressure boundary leakage
- 1 gpm unidentified leakage
- 10 gpm indentified leakage
- 150 gpd primary to secondary leakage through any 1 S/G

PCS Pressure Isolation Valve Leakage
LCO 3.4.14 requirement
- Leakage from each PCS PIV shall be w/in limits and both SDC suction valve interlocks shall be operable
- Separate Condition entry is allowed for each flow path
- Enter applicable conditions and required actions for systems made inoperable by an inoperable PIV
LCO 3.4.14 applicability
- Modes 1-3
- Mode 4, except during the SDC mode of operation, or transition to or from the SDC mode of operation

PCS Leakage Detection Instrumentation
LCO 3.4.15 requirement
3 of the following PCS leakage detection instrumentation channels shall be operable:
- 1 CTMT sump level indicating channel
- 1 CTMT atmosphere gaseous activity monitoring channel
- 1 CAC condensate level switch channel
- 1 CTMT atmosphere humidity monitoring channel
LCO 3.4.15 applicability
Modes 1-4
LCO 3.4.15 required actions < 1 hour
All required channels inoperable, then enter LCO 3.0.3 immediately

LCO 3.4.17 requirement
- S/G tube integrity shall be maintained AND all S/G tubes satisfying the tube repair criteria shall be plugged in accordance with the S/G Program
- Separate Condition entry is allowed for each S/G tube
