Describe the timeline of neuroleptic ADRs:
1st few days: Acute Dystonia, Torticollis–>
1 mos: Parkinsonism–>
2 mos: Akathisia (restlessness)–>
Several mos/ years: Tardive dyskanisia
Describe Tardive Dyskininesia:
- NOT extrapyramidal SE
- IRREVERSIBLE, LATE movement disorder associated with HIGH potency neuroleptics
- Characterized by lip smacking and stuffs
What is Neuroleptic malignant syndrome?
5 Features and treatment:
- Delirium
- ANS instability
- MSK rigidity–> Myoglobinuria + Hyperpyrexia
STOP DRUG and Give DANTROLINE or BROMOCRIPTINE
Drug that causes agranulocytosis?
Clozapine: most potent atypical antipsychotic, highly effective and highly dangerous; requires weekly CBCs
