What is sleep apnoea?
This disorder is characterised by intermittent closure/collpase of the pharyngeal airway causing apnoeic (not breathing) episodes during sleep
What are the clinical features of sleep apnoea?
Loud snoring Daytime sleepiness Poor sleep quality Morning headache Decreased libido Nocturia Decreased cognitive performance
What are the complications of sleep apnoea?
Pulmonary hypertension
Type II resp failure
Hypertension
What investigations are done for sleep apnoea?
pulse oximetry
epworth sleepiness scale
Video analysis
polysomnography is diagnostic (ECG,EMG, abdo movements)
What are the grades of sleep apnoea?
5-15- mild
15-30 moderate
30> severe
How do you manage sleep apnoea?
Weight reduction Avoidance of tobacco and alcohol Mandibular advancement device CPAP (mod-severe disease) Surgery to relieve obstruction
What are the clincial features of narcolepsy?
Falling asleep during the day Cataplexy excessive daytime sleepiness sleep paralysis hypnagogic hallucinations
How do you manage narcolepsy?
Modafinil
Clomipramine (cataplexy)
Sodium oxybate
