Adult destination determination policy
The officer-in-charge will determine transport destination after considering the following: (3)
• Patients who are UNSTABLE or in CARDIAC ARREST will be transported to the closest most appropriate hospital
• Patients who are STABLE will be transported to the hospital where the patient is usually seen and has the patient’s medical records
• Special Treatment Circumstances: (3)
▪ Patients to meet stroke alert status will be transported to a State of Florida approved comprehensive or primary stroke center
▪ Patients you meet STEMI alert status should be transported to a facility with it Interventional cardiac catherization capability
▪ Patients with pregnancy related complaints will be transported to a facility with obstetric (OB) capability

Pediatric destination determination policy
The officer-in-charge will determine transport destination after considering the following: (3)
- Pediatric patients who are UNSTABLE or in CARDIAC ARREST should be transported to the closest approved Pediatric Emergency Department
- Transfer to an alternate hospital should only take place if the hospital is closer to the scene of the incident in the patient’s immediate condition is such that the patient’s life will be in danger if care is delayed by proceeding directly to a an approved pediatric emergency department
- Patients who are STABLE should be transported to the hospital where the patient is usually seen and has the patient’s medical records
- STABLE pediatric with a simple injury or complaint and no hospital affiliation: transport to the closest approved Pediatric Emergency Department
- STABLE pediatric with a complex complaint and/or medical history (i.e. congenital cardiac neurological abnormalities) transport to Baptist Downtown or UF Health.

Non-transport situations
The following situations will not require transport of patient to hospital by rescue. (Must complete signed AMA or non-emergency/non-transport transport refusal) (7)
- Situations where no emergency exists
- Injuries of a superficial or minor in nature
- Situations where transport would endanger the safety of rescue crew members
- Patients, otherwise stable, who require transport to mental health resource or Detox facility
- Situations where patients request private provider or private vehicle transportation
- Patients refuse transportation
- DOS victims will not be transported

Transport after administration of medications (3)
- Patients will not be giving medications in the field and then released or allowed transport by private vehicle
- Prior to medication administration, patient condition permitting, patient should be informed that if JFRD personnel administer medication, the patient must be transported by JFRD for physician evaluation
- Even if the patient’s condition improves, the patient should still be transported by rescue for evaluation

Family, friends or other passengers will not be routinely transported UNLESS REQUIRED for the continuation of patient care.

Patients will be transported by rescue (with proper consent)
The following situations require transport: (17)
• Life threatening emergencies in those situations which could become life-threatening
• Chest pain/cardiac signs and symptoms/epigastric discomfort
• Systolic BP > 180 mmHg OR Diastolic BP > 110 mmHg
• Abdominal pain
• Reported loss of consciousness
• Report of blood in stool or vomit
• Severe sudden onset headache, light intolerance and/or neck stiffness
• Difficulty breathing
• Pregnancy
• Diabetic emergencies
• Altered mental status
• All drowning patients
• Patients to meet trauma, STEMI or stroke alert status
• Patients who require or have received Advanced Life Support treatment and or medications
• Conditions that might be exacerbated by improper handling or inappropriate transport
• Pediatric patients 15 years of age or younger who are symptomatic
▪ Exceptions are superficial injuries when a competent parent or legal guardian is on scene or documented sign refusal of treatment or transportation
• Elderly patients 65 years of age or older
▪ Exceptions are non-symptomatic confident patients without a documented chief complaint or documented signed refusal treatment/ transportation

Patients transported by Rescue will normally be transported to the emergency department.
• Exceptions (4)
- Obstetric patients may be taken directly to the labor and delivery from ED when accompanied by an RN, PA or physician
- Obstetric patients may be taken directly to Baptist Pavilion, Baptist Beaches OB or St. Vincent’s Southside Women’s Center after consultation with the ED via radio
- STEMI alerts may be taken directly to the cardiac catherization lab when accompanied by an RN, PA or physician
- Inter-facility transfers may be taken directly to the appropriate department. Transferring physician will determine the transport destination
