How do nitrates work?
Prodrugs which are converted to nitric oxide
Cause a reduction in intracellular Ca, resulting in vasodilation
What does a low dose of nitrates cause?
Reduction in pre-load, reducing the work of the heart
What does a high dose of nitrates cause?
Vasodilation of coronary arteries
What issue is associated with the prescription of nitrates?
Tolerance develops quickly: a nitrate free period must be provided every 24 hours
What methods of delivery can be used for nitrates?
Sublingual
PO
Cutaneous
IV
What side effects are associated with nitrates?
Headache, hypotension
Interact with phosphodiesterase 5 inhibitors causing severe hypotension
How do beta blockers work?
Block action of adrenaline and noradrenaline on B1-adrenergic receptors (cardioselective)
Reduce oxygen demand by decreasing HR, BP, contractibility
What is the effect of B1 receptors?
Increase heart rate and contractability
What is the effect of B2 receptors?
Dilate blood vessels and bronchi
What is the effect of a1 receptors?
Vasoconstriction
What are the side effects of beta blockers?
Bronchoconstriction Bradycardia Precipitate heart failure Exacerbate peripheral vascular disease Alter lipid profile
