101 - SAFETY FUNDAMENTALS Flashcards
101.1
Explain these parts of the five step process as they apply to ORM:
Identifying hazards
Assessing hazards
Making risk decisions
Implementing controls
Supervising
Identifying hazards: Condition with the potential to negatively impact mission or cause injury, death, or property damage.
Assessing hazards: Determine the associated degree of risk in terms of probability and severity.
Making risk decisions: Idintify controls and decide how to proceed.
Implementing controls: Implament controls to minimize risk.
Supervising: Determin effectiveness of controls and adapt as needed.
101.3
Define “high-risk” course.
Training with an elevated probability of loss or an increased level of severity is likely or imminent.
101.4
Discuss what type of courses are considered “high-risk”.
Training that exposes staff, students or assets to the potential risks of death, permanent disability, or loss.
101.5
Discuss the difference between these types of courses.
Voluntary
Involuntary
Voluntary: has the ability to Drop On Request and return to original rating. Identified by a special course indicator code of “A” or “D” in CeTARs.
Involuntary: Accession training or follow-on specialty skills school. Can not drop.
101.6
Discuss what Drop on Request “DOR” is and the procedures for conducting one.
Student says “I quit”.
Student removed from the training.
A summary is entered in the student’s record and the command’s permanent records.
101.7
Discuss what Training Time Out is and the procedures for conducting one.
When students or instructors have concern for safety they call a “TTO.”
Training will immediately cease until the situation is returned to a safe state.
Only then will training resume.
101.8
Discuss the purpose of the EAP and how often must it be reviewed for accuracy of information. Identify the periodic requirements for fully exercising the EAP. How often is it to be fully exercised?
Internal plan to be implemented immediately upon advent of a mishap to aid involved persons and to control and/or safeguard the scene.
Conduct quarterly walk through and fully exercise annually.
101.9
Discuss periodic requirements for safety stand-downs.
Once a year or at COs discretion.
101.10
Discuss the following as it pertains to “high-risk” instructor.
Mishap reporting
Site Augment Plans
Special Course Indicator Code
Core Unique Instructor Training
Instructor Screening Process
Evaluation Process
Training Safety Officer
Mishap reporting: Report mishaps and injuries.
Site Augment Plans: Unique training situations for high risk courses. Developed by the CCA.
Special Course Indicator Code: Voluntary high risk training that is identified by a special course indicator code of “A” or “D” in CeTARs.
Core Unique Instructor Training: Prepare the instructor to teach in a high risk course.
Instructor Screening Process:
- Service Record Screen.
- Physical Requirements.
- Medical Officer Interview.
- COs Interview.
Evaluation Process: Evaluate based on amount of time spent instructiong in spacific area.
Training Safety Officer: Be familiar with high-risk training procedures. Ensure all training mishaps are investigated. Keep the CO or OIC advised of all training mishap investigation results.
101.11
Discuss the importance of heat/cold stress, monitoring, and control in a training environment.
Ensure sailors are prepared to participate in the activity and mitigate potential loss of life.
