What is Population Health
A
Population - Set of people who have common personal/environmental characteristics
- Characteristics include age, race, sex, social class, medical diagnosis, level of disability, exposure to toxin, participation in health seeking behavior (smoking cessations).
- The researcher/HCP identifies the characteristics that make up a population
Population Health
- Health outcomes of a group including distribution of outcomes within the group
- Outcomes of driven approach involves tracking health indicators (not necessarily based on geographical area of certain groups.
Population Health Programs
- Focus on at-risk populations. Health programs include increasing childhood vaccinations, reducing teen pregnancy rates, decreasing smoking amongst congestive heart failure patients

Public Health vs Population Health
A
- Public Health involves approaches used to improve health of a population that anyone can use, not just healthcare workers

Community Health vs Population Health
A
- Community health has the same goals as population and public health but is organized in a geographical area
- More heavily involved in local government and policy
- Uses public health science and EBP to work with communities in a culturally appropriate manner to optimize health and quality of life
