10.20 General (Standard Reports)
A. All incidents, on or off duty, where a CO acts in a manner that requires a report, SHALL do so in writing, using the STANDARD REPORT FORM. 🚨
B. The first CO responding to an incident, unless otherwise directed by supervisor, SHALL be RESPONSIBLE for the proper preparation / submission of appropriate report form. 🚨
C. Reports SHALL be submitted as DIRECTED by supervisor but NO LATER than before the conclusion of CO’s next tour of duty. 🚨
10.20 General (Standard Reports)
E. Incidents that involve judicial threats or involve court officers, such as:
Reports of arrests, discharge, or loss of firearms, the use of force, DV-related offenses, or other incidents that require immediate attention.
Chief Court Officers, Majors or Captains MUST submit the reports (NO LATER than one week after the filing of report by CO) to the Div.of Court Operations, Court Security Unit and will be reviewed by the Chief of Public Safety.
10.30 (UF-100) Aided Report
A. (UF-100) AIDED REPORT🚨
SHALL be completed when responding to aided cases, but not be limited to:
- A sick or injured person.
- A mentally ill / emotionally disturbed person. (EDP)
- A deceased person.
- A lost person.
- A neglected, abandoned, destitute, or abused child.
- A maternity case.
B. (UF-100) AIDED REPORT form, CO SHALL complete as follows: 🚨
- Complete all boxes with captions about the situation and Use military time.
10.30 (UF-100) Aided Report
B. (UF-100) AIDED REPORT form, CO SHALL complete as follows: (Cont.)🚨
- If more than one card needed, mark (No.1, No.2,) cards on the upper right front.
- Under the caption “Nature of illness or injury,” use your own words to describe the apparent illness or injury, such as “chest pains,” “head injury,” “maternity,” etc.
- If aided refuses medical attention, check the RMA box under the caption “removed to.”
- Provide details in short narrative form.
- Submit the completed report to the Major or Captain.
10.30 UF-100 Aided Report
(UF-131) MEDICAL WAIVER FORM - RMA🚨
C. (UF-131) MEDICAL WAIVER FORM🚨
SHALL be completed from ANY individual who REFUSES MEDICAL ATTENTION. If aided REFUSES to SIGN the UF-131, must note in report.
10.30 UF-100 Aided Report
D. The Major/Captain SHALL check Aided Report card for accuracy and completeness, assign a code and aided number and fwd the original to the Div. of Court Ops, Court Security Unit.
- and copies to the Chief Clerk / Executive Assistant and Chief Court Officers and SHALL retain a copy in their office.
10.40 UF-101 Unusual Occurence Report
C. (UF-101) UNUSUAL OCCURENCE REPORT
SHALL be completed when responding to, but not limited to, the following: 🚨
- ON -DUTY:
a. Prisoner escape
b. Attempted escape
c. Bomb threat
d. Disruptive prisoner
e. Disruptive spectator
f. Report of crime
g. Property theft
h. Arrest
i. Summons
j. Use of force
k. Stop and Frisk
10.40 UF-101 Unusual Occurence Report
C. (UF-101) UNUSUAL OCCURENCE REPORT SHALL be completed when responding to, but not limited to, the following: 🚨
- OFF-DUTY:
a. CO using authority derived as a result of being a peace officer (arrest, use of force).
b. CO arrested / charged with a crime.
c. CO subject of an order of protection.
d. Injuries that may affect ability to perform CO duties.
e. Any situation that may affect the officer’s employment with the Unified Court System.
10.41 Magnetometer / X-Ray Post Records
A. (UF-126) MAGNETOMETER CALIBRATION RECORD: 🚨
1. Completed DAILY and maintain no less than one year. (Quiz)
B. (UF-113) WEAPONS AND CONTRABAND REPORT: 🚨
1. Completed MONTHLY fwded to Div. of Court Ops and be retained for five years. (Quiz)
10.41 Magnetometer / X-Ray Post Records
C. (UF-135) PROPERTY RECEIPT: 🚨
1. RECORDS are retained for two years.

10.43 Jury Reports
A. (UCS-115) JURY VOUCHER 🚨
SHALL be completed by Sgt. or designee when required to provide lodging, meals, and transportation for sequestered jurors.
B. (UCS-116) JURY SEQUESTRATION LOG 🚨
C. For Sequestered Juries:
(UF-144) OVERNIGHT JURY REPORT 🚨
(UF-144A) JUROR NOTIFICATION FORM 🚨
10.44 Leave Requests
A. LEAVE REQUESTS:
Shall be retained in Ops for six months.

10.45 Infectious Disease Cases Reports
A. (UF-136) INFECTIOUS DISEASE CASE REPORT 🚨
- SHALL be completed when CO receives a notification that a litigant has an infectious disease - retained in Ops until completion of case.
10.50 Coding
- Aided and Unusual Occurrence Reports shall be CODED in the order below:
A. Location Code (MUST include a LETTER, may have a number)
B. Report Number (MUST have 3 digit, sequential order)
C. Year (MUST be 2 digits)
