When were talking about language were talking about
The brain
-Conception, formulation, memory for syntax and semantics
-understand that phonemes have meaning
making language decisions understudying that language has phonemes and when the child cannot produce them they will compensate and we will have to understand them
When were talking about speech were talking about
- Brain for motor for the movement in speech
- Sensory for the feedback about your tongues, teeth and lips, the actual physical mechanism and then pathways)
T/F Cognitive effects are common in a syndrome
this will make a difference to language development (this cannot be separated!)
T/F isolated cleft palate kiddos catch up once the mechanism is repaired
Langauge wise, although their delayed because they cannot properly practice with the physical mechanism, they are NOT cognitively delayed
vs. children with syndrome cannot catch up as easily because of cognitive delay
What are obligatory errors?
-Correct place, but not voice or manner
-all speech mechanism errors not language based
the teeth, closure, lip
-cannot close velopharyngeal port
ex. Nasal Air Emission (NAE)
ex. Hypernasality
-Air escaping in odd places because of dentition
substitutions because they have a cleft
T/F compensatory errors are the most common
- Incorrect place, voice, or manner
- Substitutions because of cleft
ex. glottal stop for frontal stop - Something I make a decision about, it’s a language and phoneme decision
- This is deliberate
what is the % of Cleft palate effects on language development
- If it is a hard palate cleft w out cleft lip is it 50/50 chance
- roughly 70% of the time the surgeries are successful
(Reminder test) when is it typical for a cleft palate repair? when does a cleft palate form ?
12 to 24 months is typical time for cleft palate repair ( Hopefully no compensatory strategies have developed if it is performed this early)
By 9 weeks the palate must form
What are recommendations for parents?
- all the verbal play is missing and they are quiet babies
- when children stop responding parents stop initiating so tell them to TALK
- KEEP TALKING even if they’re not giving you a lot feedback
- expose them to new experiences and words
- they don’t get practice in consonant inventory but -they need to be exposed to it
- they need to develop vocal and syntax—phoneme collection
