Zollinger Ellison syndrome Flashcards
What is it
a rare endocrine disorder with 3 main parts:
- gastronoma (gastrin secreting tumour - increased gastrin causing peptic ulcers)
what is gastrin?
released by G cells in the
what does gastrin do?
stimulates the parietal cells to secrete hydrochloric acid and increases cell division on the epithelial layer
what does Brunner glands secrete?
mucous filled of bicarbonate to neutralise the acid.
what happens in Zollinger Ellison syndrome?
G cells get a mutation and divide uncontrollably in the stomach or duodenum into gastronomas - these can be benign or malignant.
The increase in gastrin causes and increase in parietal cells secreting hydrochloric acid which causes erosion and peptic ulcers.
where do ulcers occur?
more likely in the duodenum.
what are the symptoms?
epigastric pain bloating belching vomiting gastric ulcer pain - during eating duodenal ulcer pain - decreases while eating Diarrhoea steatorrhoea
what is diagnosis?
peptic ulcers duiring endoscopy
high levels of gastrin in the blood, especially high levels of gastrin after giving secretin - which is meant to inhibit gastrin release.
Treatment?
Medication to reduce Acid
Surgery to remove tumour
where do gastronomes occur?
Mostly seen in the pancreas but can occur in the duodenum or stomach
