What are the two approaches to dysarthria intervention?
- Global approach
- Sub-system approach
What is a global approach?
An approach that targets one or more of the speech subsystems, but has a single treatment emphasis
Has broad reaching effects across multiple subsystems
Uses simple, easy to understand cue
Helps clients to adjust speech in several ways without having to be conscious of it. For example, Be loud = changing posture, deeper breathing, greater vocal fold adductions, changes in articulation
What are the most common global approaches?
Speak Out, LSVT, Be Clear
How does Lee Sliverman Voice Treatment work (LSVT)?
Used for people with Parkinson’s Disease
Singular focus = be loud
Designed to increase vocal intensity in client’s with PD (mild-moderate)
Lots of feedback
Incorporates principles of motor learning
Can maintain for 12-24 months with daily activities

How does Be Clear work?
Used for patient’s with nonprogressive dysarthria
Singular focus= be clear
Purposeful enunciation/over-articulation of all sounds and rate reduction
Goals: improve intelligibility and improved self-perception of speech
How does Speak Out work?
Use for people with Parkinson’s disease
Singular focus = speak with intent
Increased attention to speech - converts speech from an automatic function to an intentional act
Speak more clearly, slowly, loudly
Who are good candidates for a global approach?
- Can follow simple demands
- respond to cueing and modelling
- Tolerate 40-60 minute sessions
- Complete daily exercises
- Commit to treatment schedule
