What are the exclusions for otic disorders?
- Signs of infection
- Pain associated with ear discharge
- Bleeding or signs of trauma
- Presence of ruptured tympanic membrane
- Ear surgery within the previous 6 weeks
- Tympanostomy tubes present
- Incapable of following proper instructions for use
- Hypersensitivity to recommended agents
<12 years of age (Impacted Cerumen Only)
How could we assess otic disorders?
- Limit OTC to minor symptoms
- Limit for 4 days of OTC
- See a primary care provider if ear pain or discharge starts during self-treatment
What is patho of impacted cerumen?
Disruption of the normal flow of cerumen toward the outer ear canal
What are the presentation of impacted cerumen?
- Sensation of ear fullness
- Dull pain or discomfort
- Itching
- Hearing loss
- Tinnitus (ringing in the ears)
- Dizziness
- Cough
What are the treatment goals for impacted wax?
Remove cerumen using safe and effective products while preventing adverse events
What are the non pharm of impacted wax?
Curette (Clinere Ear Cleaners)
What are the pharm treatments of impacted wax?
- Creamed peroxide
- Docusate Sodium - off-label use
- Hydrogen Peroxide 3% - off-label use
- Olive Oil (Sweet Oil) – off-label use
What is the dosing of carbide peroxide?
Dosing: 5-10 drops in affected ear for several minutes (up to 15) then irrigate with warm (NOT Cold or Hot) water with a bulb syringe; Use twice daily for up to 4 days
What is the patho of water clogged ears?
Retained water in ear canla
What is the presentation of water clogged ears?
- Hearing impairment
- Discomfort
- Sensation of fullness
What is the non pharm of water clogged ears?
Prevention (e.g., ear plugs, swimming cap, gently blow drying the ear canal, etc.)
What is the pharm for water clogged ears?
1.Isopropyl Alcohol 95% in Anhydrous Glycerin
2. Acetic Acid 2.5%
