1.0 mg q 3-5 minutes

Hypovolemic shock

What does lidocaine do?
Increases fibrillation threshold
Decreases ventricular irritability
When should you use lidocaine?
Second line drug for v-fib, pulseless v-tach
Optional for v-tach with pulse
First line for v-ectopy
What is the dose and route of lidocaine?
1.0-1.5 mg/kg
Max dose is 3.0 mg/kg
IV IO and ET
What are the contraindications for lidocaine?
Heart block
Bradycardia
Adam stokes syndrome
What are the side effects of lidocaine?
Lightheadedness
Blurred vision
Numbness
Hypotension
What are the special notes for lidocaine?
Overdose toxicity causes seizures.
Cut max dose in half for >70 yrs old or anyone with renal or hepatic failure.
What drug class is atropine sulfate?
Parasympatholytic
What does atropine sulfate do?
Enhances sinus automaticity
Enhances AV conduction
When do you use atropine sulfate?
First line for bradycardia
First line for heart blocks
Organophosphate poisoning
What is the dose and route of atropine sulfate?
0.5 mg rapid IV or IO (ET normal speed)
Max 3.0 mg adult
Max 0.04 mg/kg pedi
What are the contraindications for atropine sulfate?
Tachycardia and narrow angle glaucoma
What are the side effects of atropine sulfate?
Tachycardia and dysthymia
Can cause reflex bradycardia if given too slow or in insufficient quantities.
What is a trade name for adenosine?
Adenocard

Antidysrhythmic
What does adenosine do?
Interrupts re-entry pathways
Slows av conduction
When do you give adenosine?
First line for SVT (Supraventricular Tachycardia)
What is the dose and route for adenosine?
6 mg rapid IV or IO
- if ineffective -
12 mg rapid IV or IO with a max of 18 mg
What are the contraindications for adenosine?
Heart blocks and SSS (Sick Sinus Syndrome)
What are the side effects of adenosine?
Chest pain
Shortness of breath
Nausea
What are the special notes for adenosine?
Transient period of asystole - no longer than 10 seconds.
