What is the intent of the information on the pink medical history sheet?
TO identify medical problems that may modify or preclude dental treatment, or require special management
For items that require a ‘yes’ answer on the medical history, what should go with it?
A written clarification that includes the date the problem occurred and/or was resolved, and the disposition of the problem (if it will factor in dental treatment)
What are the 4 items that should go in a patient’s biological data?
Age
Ethnicity
Gender
Occupation
What is the difference between chief complaint, and the present illness?
Chief complaint = patient’s own words
Present illness = more detailed description of patient’s chief complaint (tooth/area, onset, duration, severity, able to get relief)
What is the abreviation to indicate that patient has no known drug allergies?
NKDA
How do you correct an error in a patient record?
Draw a single straight line through the error, and provide initials, date, and add the correction.
