W4 D2 - Pancreatitis Flashcards
Where is the pancreas located?
On the left side partially under the liver shaped like a fish
Describe the location of the pancreatic head, tail, main pancreatic duct, sphincter of Oddi, common bile duct
head
* thicker end at midline by the duodenum
tail
* narrow tip on the left side
main pancreatic duct
* keeps digestive juices between pancreas and duodenum
sphincter of Oddi
* A muscle that opens and closes to allow bile / pancreatice juices into the small intestine
common bile duct
* carries bile from liver to gallbladder
Describe the size and function of acini cells, islets of langerhans, beta cells, alpha cells
acini cells
* large cells on the outside
* releasing enzymes to aid in digestion
islets of langerhans
* small cells inside
* endocrine portion of pancreas
beta cells
* small cells inside
* insulin and blood sugar shifting into the cells
alpha cells
* small cells inside
* dump glucogon into the capillary
What are the functions of the pancreas?
Exocrine - to outside of the body
* sodium bicarbonate maintains pH
* pancreatic enzymes
Endocrine - inside the body / into the blood
* insulin
* glucagon
How does the pancreas aid in glucose control?
What is pancreatitis?
An autodigestion issue where the pancreas ‘eats’ itself
What are the different types of pancreatitis?
Mild acute
* no organ failure
* resolves quickly
Moderate acute
* brief organ failure
* resolves in 48 hours
Severe acute
* persistent organ failure
* ICU admission
Chronic
* long term
* alcohol consumption or unknown
What is the pathophysiology and phases of pancreatitis?
Phase 1: early activiation of trypsinogen
* to trypsin
* begins to digest protein structures
* other enzymes become active
Phase 2: intra-pancreatic damage
* enzymes breakdown blood vessels / fat into fatty acids
* inflammatory mediators released
Phase 3: extra-pancreatic damage
* systemic affect from extensive damage
* digestive enzyme and inflammatory mediators spread to other organs
What are management goals for pancreatitis?
- identify and reverse cause
- stabilize and support care
- treat complications
