Relapse prevention plan method

A
  1. Develop a list of symptoms chronologically and colour code based on severity
    - Ask stakeholders for a different perspective
  2. Develop a list of ways to stay well
    - Based on what the individual knows helps them feel well (e.g. eating a balanced diet, regular exercise, socialising)
  3. Brainstorm a list of triggers
  4. Develop strategies for each colour of symptoms

What is a personal crisis plan?

A
  • Given to stakeholders
  • List symptoms in a way that someone else can recognise
  • Discusses people who CAN and CANNOT take over and what their role is
  • Has names and numbers (e.g. doctors)
  • Lists medications and what they are used for, allergies
  • What they person wants in a crisis and what they want to avoid
  • List of care options and preferred treatment facilities
  • How to tell when they no longer need help
