What is recovery?
A
Being able to live and create a meaningful life in a community of choice with or without the presence of mental health issues

Relapse prevention plan aims
A
- Maintain wellness so that a meaningful life can be lived
- Reducing the recurrence of illness and strengthening functional capacity
- Empower people to take ownership of their care and strive for independence

Relapse prevention plan method
A
- Develop a list of symptoms chronologically and colour code based on severity
- Ask stakeholders for a different perspective - Develop a list of ways to stay well
- Based on what the individual knows helps them feel well (e.g. eating a balanced diet, regular exercise, socialising) - Brainstorm a list of triggers
- Develop strategies for each colour of symptoms

A
- Program run by peers that has a focus on self management, determination and hope

What is daily maintenance?
A
- What am I like when I’m well?
- What do I need to do every day/month to feel alright?

What are triggers?
A
- External events or circumstances that are risk factors for wellness
- Develop a plan to prevent/avoid triggers where possible and ease the effects of triggers

What are EWS?
A
- Develop a list of these when things are starting to go down hill and develop a plan for them

What to do when things are breaking down?
A
- Develop a list of signs that things are getting worse
- Develop an action plan and list their supports and strategies

What is a personal crisis plan?
A
- Given to stakeholders
- List symptoms in a way that someone else can recognise
- Discusses people who CAN and CANNOT take over and what their role is
- Has names and numbers (e.g. doctors)
- Lists medications and what they are used for, allergies
- What they person wants in a crisis and what they want to avoid
- List of care options and preferred treatment facilities
- How to tell when they no longer need help
