what is the dominant bacteria in our coochie and what does she do
Lactobacillus: maintains acidic pH by making lactic acid (and hydrogen peroxide) - toxic to anaerobic bacteria
estrogens role?
Stimulates proliferation and maturation of epithelium - provides a physical barrier (and produces glycogen for lactobacilli to eat and make acid)
vaginitis
any inflammation of vagina result in itching, discharge, or pain
how do you do a wet mount?
obtain species from speculum exam
place specimen on slide and apply 1 drop of saline
place a cover slip over specimen on slide
prepare a second slide & instead of saline add KOH
review under microscope
normal vaginal discharge
wet mount: epithelial cells
color: white
viscosity: high
consistency: floccular
bacterial vaginosis
odor!!!! wet mount: clue cells color: gray viscosity: low consistency: homogenous presence in vag: adherent to walls
trichomoniasis
increased discharge wet mount: trichomonads color: yellow-gray viscosity: low consistency: homogenous adherent to vag wall
Candidiasis
itchy & burning hyphae/spores white high viscosity floccular consistency adherent to vag walls
i have candida, how do you treat me?
Miconazole 2% cream or 100-200mg suppository
Fluconoazole 150mg PO
I have BV, treat me
.75% metronidazole topical intravaginal/week
Metronidazole 500mg PO/week
2% clindamycin cream intravaginal/week
i have trichomoniasis, treat me
Metronidazole 2 G PO 1 dose (more effective)
Metronidazole 500mg PO/week
