Treatment plan for Vitamin K (Phytonadione)

A

Vitamin K 1-10mg po/IV x1
Sutra therapeutic INR with no bleeding:
INR 4.5-10 Vit K 2.5mg po x1
INR >10 2.5-5mg Vit K po x1

Supratherapeutic INR w/ bleeding
Minor bleeding: 2.5-5 po x1
Major bleeding 10mg IV x1

Chronic malabsorption: 1-2 mg po QD
