A

What are the requirments for a vet prescription?

A

The prescription must include:

* The name, address and telephone number of the person prescribing the product 
* The qualifications of the person prescribing the product;
* The name and address of the owner or keeper; 
* The identification and species of the animal or group of animals to be treated;
* The premises at which the animals are kept if different from that of the owner or keeper
* The date of the prescription;
* The signature of the prescriber
* The name and quantity of the product prescribed;
* The dosage and administration instructions (the dose must be stated, a dose of ‘as directed’ is not acceptable);
* Any necessary warnings; and
* The withdrawal period if relevant.
* If it is prescribed under the cascade, a statement to that effect
