What are the schedules for CD?
Sch 1-5
what is CD sch 1?
most sch 1 have no theraputic use, and a licensce is required for their production, possesion or supply
what CD sch 2?
includes opiates (eg: diamorphine, morphine, methadone, oxycodone, pethidine, lisdexamfetamine, methylphenidate
what is CD sch 3?
buprenorphine, temazepam, tramadol, midazolam, phenobarbital, gabapentin, pregabalin
what is CD sch 4?
sch 4 is split into two parts:
- Part 1 most of the benzodiazepines (diazepam), non-benzodiazepines (zoplicone, aleplon, zolpidem tartrate,) sativex (oral mucosal spray)
- Part 2 androgenic and anabolic steroids, clenbuterol, chorionic gonadotrophin (HCG), non-human chorionic gonadotrophin, somatotropin, somatrem, and somatropin. Controlled drug prescription requirements do not apply and Schedule 4 Controlled Drugs are not subject to safe custody requirements.
Records in registers do not need to be kept (except in the case of Sativex®)
what is CD sch 5?
includes preparations of certain Controlled Drugs (such as codeine, pholcodine, or morphine ORAL SOLUTION) which due to their low strength, are exempt from virtually all Controlled Drug requirements other than retention of invoices for two years.
how is a precription for CD 1-4 valid for?
28 days
how is a precription for CD 5 valid for
6 months
what sch does the precriber have to be in the UK?
Sch 2&3
Are sch 2&3 prescriptions repetable?
No, but 4 and 5 are
is ES allowed for sch 2&3?
No, ES only allowed for sch 4&5
Which CD should retain the invoice for 2 years?
Sch 3 and 5
what are the legal requirments on a CD requisition form?
Signature of the recepient
name of the recipient
address of the recipient
profession or occupation
total quantity of drug
purpose of requsition
processing a requisition form
-mark the requisition indelibly with the suppliers name and address
-pharamcy stamp
send the requisition to relevant NHS agency
what can midwife obtain using a midwife supply order?
diamorphine, morphine and pethidine
CD prescription requirments
- Prescriber signature - indelible ink
- Date - only valid for 28 days
- Prescribers address (UK)
- Dose
- Formulation
- Strength –if more than one available
- Total Quantity – in both words and figures
- Acceptable quantity prescribed (30 days)
- Patient name

when should the first instalment on a instalment prescription be dispensed by?
no later than 28 days after the appropriate date

Name, sign, date and GPHC no
What should a private CD prescription be on?
FP10PCD, but vet prescription doesnt not have to be
