What are the legal requirements for a veterinary prescription?

A

Name, address, telephone number, qualification + signature of prescriber
- If CD 2 + 3, RCVS reg number must be included

Name + address of owner
Identification + species of animal

Address if different from owner

Date
- Valid for 6 months
- CD 2, 3 + 4: valid for 28 days

Name, quantity, dose + admin instructions
- ‘as directed’ NOT acceptable

Warnings + relevant withdrawal period (time btw animal receive med + when it can be used for food)

‘Prescribed under the veterinary cascade’ - human/unlicensed

If repeatable, number of repeats

If cd: ‘the item has been prescribed for an animal or herd under the care of the veterinarian’ + usual CD requirements
