UTIs Flashcards
UTI tests
Urine dipstick/UA URI-cult CLED/EMB Gram stain Culture Abx susceptibility
CLED/EMB
QUANTITATIVE - CFU/mL
EMB - Selective for G-
CLED - Determines ability to ferment
CFU/mL consistent with UTI
5x10^4
Cells seen on HPF of UTI specimen
2-5 WBC OR
15 bacteria
UTI pathogens
E. coli Klebsiella spp Proteus spp Enterococcus spp Staphylococcus saprophyticus Streptococcus (Group B) Pseudomonas aeruginosa
Most common cause of UTI
Uropathogenic E. coli (UPEC)
E. coli morphology
Gram negative
Flagellated
Encapsulated (K1) strains = meningitis
UPEC transmission
Fecal contamination
UPEC virulence factors
P fimbriae (pili)***
Dr adhesins
Alpha & beta hemolysins
K (capsular) antigen - typically seen in upper or chronic UTI
Proteus morphology
Gram negative
Bacilli
Possess fimbriae
Proteus species that cause UTI
P. vulgaris (immunocompromised)***
P. mirabilis (community acquired)
P. vulgaris diagnosis
- “Swarming” on culture agar (CLED prevents swarming)
- Produce UREASE (unique)
Struvite stones
Magnesium ammonium phosphate
Unique to P. vulgaris (due to urease activity)
S. saprophyticus morphology
Gram positive
Coagulase negative
Staphylococcus
S. saprophyticus key notes
- Normal flora of female genital tract
- SECOND leading cause of UTI
- Prominent in females ages 17-27 (sexually active)
