E. coli
Define lower UTI
An infection of the bladder (also known as ‘cystitis’), usually caused by bacteria from the gastrointestinal tract entering the urethra
UTI Sx
Urinary Sx
- Dysuria
- Haematuria
- malodourous
- cloudy
- increase urine freq.
- burning sensation
- suprapubic pain
UTI Ix
- Urine dip
- Urine MC&S - MSU
UTI Mx + choice of Abx for non-preg
- Nitrofuratoin 100mg BD 3/7
- Trimethoprim 200mg BD 3/7
UTI Mx + choice of abx for preg women in the 1st/2nd trimester
Nitrofurantoin 100mg BD 7/7
Nitrofuratoin contraindications in pregnancy
- Avoid in the 3rd trimester - neonatal haemolysis
Trimethoprom contraindications
can cause isolated rise in Cr level
Nitrofurantoin contraindications in renal impairment
avoided in pt with egfr < 40
Urine MC&S as 1st line Ix indications
- Pregnancy
- Aged > 65
- not improving w/ abx
- Recurrent UTIs
- Urinary cathetaer in-situ
Choice of abx for UTI in 3rd trimester of pregnancy

Cefalexin 500 mg BD 7/7
E.coli gram staining findings
Gram -ve (Pink)
Rod or Bacillus
Anaerobe
UTI complications
- Pyelonephritis
- Urosepsis
UTI in men Mx
- Nitrofuratoin 100mg BD 7/7
OR - Trimethoprim 200mg BD 7/7
