Polycystic Kidney Disease Flashcards
Define Polycystic kidney disease
a genetic condition where the healthy kidney tissue is replaced with many fluid-filled cysts.
which gentic pattern is more common in PKD
Autosomal dominant
2 PKD genes
Affected genes in autosomal dominant PKD
- PKD1 gene on chromosome 16
- PKD2 gene on chromosome 4
Extra-renal manifestations of PKD
- Berry aneurysm formation (SAH)
- mitral valve disease (regurg)
- liver, splenic, pancreatic, ovarian and prostatic cysts
- colonic diverticula
Autosomal Recessive PKD affected genes
PKHD1 on chromosome 6
ADPKD diagnostic criteria for age 15-39
three bilateral renal cysts
ADPKD diagnostic criteria for age 40-59
two cysts in each kidney
ADPKD diagnostic criteria for age > 60
four cysts in each kidney
Most common extra-renal manifestation of PKD
Hepatic cysts
PKD pharmacological Mx
- Tolvaptan - slow the development of cysts and the progression of renal failure
- Antihypertensives (ACE-i) - to control HTN
- Analgesia - pain control
- Abx - infected cysts / UTI
PKD surgical Mx
- Drainage - by aspiration or surgery
- Dialysis - for ESRF
- Renal transplant - for ESRF
PKD choice of imaging
Renal / KUB USS
Choice of imaging for cerebral aneurysm screening in PKD
MR Angiography
PKD general advice for pt
- Avoid contact sports - to prevent cyst rupture
- Avoid NSAIDs / anticoagulants
- Genetic counselling
Tolvaptan drug class
vasopressin receptor antagonist
