symptomatic but no bacteriuria
Classification of UTI
- Uncomplicated - normal GU tract
- Complicated- abnormal GU tract, obstruction, decreased renal function etc..
- Recurrent - new organism
- Relapse - same organism
Pyelonephritis symptoms
Fever
Loin pain
Vomiting
Oliguria
Cystitis symptoms
Frequency/urgency Polyuria Haematuria Dysuria Suprapubic tenderness Foul smelling urine
Prostatitis symptoms
Flu-like symptoms
Low backache
Dysuria
Tender swollen prostate on PR
Sterile pyuria definition
Increased white cells in urine but no bacteria
Sterile pyuria causes
TB Treated UTI Appendicitis Calculi Papillary necrosis Polycystic Kidney Chemical cystitis (e.g. cyclophosphamide)
UTI risk factors
Female Sex Pregnancy Menopause Diabetes mellitus Abnormal tract: stone, obstruction, catheter, malformation
UTI organisms
E. coli
Staphylococcus saprophyticus
Proteus (alkaline urine → struvite renal stones)
Klebsiella
UTI investigations
Dipstick
Midstream urine for microbiology
Bloods: FBC, U+E, blood cultures (if systemic signs)
US: children, men, recurrence, pyelonephritis
Cystitis treatment
Treat for 3-6 days Trimethoprim Nitrofurantoin (not in Renal Failure) Cefalexin (good in Renal Failure) Co-amoxiclav
Pyelonephritis treatment
Cefotaxime for 10 days
No response: Augmentin + gentamicin
