Submucosal (penduculated) fibroids found
Protrude into the uterine cavity
in the endometrium
usually just palpable as an enlarged uterus
May cause heavy bleeding
Subserosal myoma
bulge through outer uterine wall
under the uterine serosa OUTSIDE the uterus
may be easily palpated on abdominal exam
Intraligamentous fibroids
Found within the broad ligament
Interstitial (Intermural) fibroids
Stay within the uterine wall
within the myometrium
lead to an irregular contour
Stages of endometriosis
Stage 1- minimal
few implants and no adhesions
Stage 2- mild
superficial implants, no significant adhesions
Stage 3- moderate
superficial and deep implants AND notable adhesions
Stage 4- Severe
multiple implants, dense adhesions, association with infertility
Endometriosis objective findings
may be normal
cervical motion tenderness CMT
bilateral or unilateral adnexal tenderness
tender nodules on the uterosacral ligments
ovarian enlargement
fixed or retroverted uterus
Forms of Adenomyosis
diffuse (2/3rds of cases)
endometrial tissue is found through the myometrium
focal
one or nodular lesions (adenomyomas) in the myometrium
Common subjective sx adenomyosis
many are asymptomatic
heavy menses
dysmenorrhea
Objective signs of adenomyosis
uterine enlargement
Tender uterus on bimanual
