Contraindication for hormone therapy
undiagnosed abnormal vaginal bleeding
known, suspected, or history of breast cancer
known or suspected estrogen-dependent cancer
current or hx of DVT or PE
current or recent (within last year) stroke or MI
liver disease
known or suspected pregnancy
known hypersensitivity to ET/EPT

for women using SYSTEMIC estrogen, a progestin is needed
to protect from endometrial hyperplasia
Dosing regimen
trial and error for individual people
strat with a low dose and titrate upward PRN
Rx the lowest dose of HT that effectively manages symptoms for the shortest time possible
optimally, EPT should not be taken for longer than 5 years
