Clinical featurs of UTI
Dysuria, increased frequency, urgency, abdominal pain, nonspecific symptoms
Which group are complications of UTIs most likely to arise in
Elderly, pregnanct women, those with foreign bodies
Causes of UTI
Most commonly E.coli, but can beother gram negative bacterias
Diagnosis of UTI
Urine dipstick positive for leucocytes and nitrites
Treatment of UTI
Nitrofurantoin or trimethoprom. Gentamicin or ciprofloxacin in severe cases
Clinical features of pyelonephritis
Fevers, malaise, loin/flank pain, vomiting, tenderness, symptoms of sepsis and LUTI
Complications of pyelonephritis
Renal damage, chronic pyelonephritis
Which groups of patients is pyelonephritis most likely to cause complications in
Pregnancy, females, diabtics and intrumentation of UT
Diagnosis of pyelonephritis
Urine dip positive for nitrites and leucocytes. FBC has raised WCC. U&E’s may show renal failure. Blood cultures. US to look for hydronephros
Treatment of pyelonephritis
IV abx in severe cases - ciprofloxacin, quniolone or gentamicin
