investigations for UTI?
- Urine sample (not routinely used in elderly or catheterised patients due to false positive)
- Urine culture/ microscopy
Female lower UTI treatment
1st= Nitrofurantoin (100mg)
2nd= Trimethoprim (200mg)
for 3 days!
PREGNANT:
1st/2nd Trimester= Nitrofurantoin
3rd Trimester= Trimethoprim
2nd line- cefalexin
for 7 days!
un catheterised male UTI treatment
1st= nitrofurantoin 2nd= Trimethoprim
for 7 days!
treatment for complicated UTI/ pyelonephritis/ urosepsis
IV amoxicillin + IV gentamicin and then cotrimoxazole for 7 days
In community- Co amoxiclav or co trimoxazole for 14 days
what is abacterial cystitis?
UTI symptoms with no significant culture or sterile pyuria
-either early UTI due to trauma (honemoon cystitis) or urethritis (STD)
what is emphysematous pyelonephritis?
-necrotising infection resulting in gas accumulation in the renal parenchyma
what is emphysematous pyelonephritis associated with?
-poorly controlled DM
most common organism for emphysematous pyelonephritis?
E.Coli
presentation emphysematous pyelonephritis?
-dysuria
-fever
-rigors
-nausea + vomiting
-AKI features
-rapid progression to sepsis
+/- crepitations in flank/scrotum
investigations emphysematous pyelonephritis?
-abdominal CT
treatment emphysematous pyelonephritis?
- percutaneous drainage and antibiotics
- nephrectomy
