Urology Flashcards
What is chronic retention
urinary retention usually caused by prostatic hyperplasia
management of chronic retention
trans urethral resection of the prostate - keep catheter until procedure carried out as retention will recur
Investigation of Renal Calculi
CTKUB in all suspected cases
- hydronephrosis + infection = surgical emergency
nephrostomy, stent or ureteric catheter.
Mx of Prostate Cancer
T1-T2: watch and wait or radical prostatectomy or radiotherapy
T3-T4: Radical prostatectomy or radiotherapy or hormone therapy
Metastatic: hormone therapy
Hormone therapy for Prostate Cancer
synthetic GnRH agonist e.g. goserelin
- negative feedback on anterior pituitary
- need an initial cover with antiandrogen to prevent an
the initial increase in testosterone
Anti-androgen e.g. cyproterone acetate
Orchidectomy
Types of renal stones
Radio-opaque - Calcium oxalate (most common) - mixed calcium oxalate/phosphate - calcium phosphate - triple phsophate Radio Lucent - urate stones - xanthine stones (rare) Semi-Opaque - Cystine stones - rare
Management of renal stones
Stone burden of less than 2cm in aggregate - Lithotripsy
Stone burden of less than 2cm in pregnant females -Ureteroscopy
Complex renal calculi and staghorn calculi - Percutaneous nephrolithotomy
Ureteric calculi less than 5mm Manage expectantly
Risk factors for erectile dysfunction
cardiovascular disease risk factors
alcohol use
drugs: SSRIs, beta-blockers
Differentials for pain in the testicle (2)
torsion
epididymo-orchitis
Differentials for swelling or lumps in the testicles(3)
hydrocoele
inguinal hernia
testicular cancer
Associations and composition of ‘Stag Horn’ Calculi
Occur due to alkalinisation of urine as a result of bacterial infection with Proteus mirablis.
Calculi contain ammonia, phosphate and magnesium (struvite)
Medical benefits of Male Circumcision
reduces the risk of penile cancer
reduces the risk of UTI
reduces the risk of acquiring sexually transmitted infections including HIV
Medical Indications for male circumcision
phimosis
recurrent balanitis
balanitis xerotica obliterans
paraphimosis
Causes of urethral stricture (4)
Iatrogenic e.g. traumatic placement of indwelling urinary catheters
sexually transmitted infections
hypospadias
lichen sclerosus
What is TURP syndrome?
Rare, life-threatening complication on trans-urethral resection of the prostate
caused by irrigation with glycine
CNS - headache, visual disturbance, respiratory symptoms and systemic symptoms.
