What is urethritis?
Inflammation of the urethra usually due to an infection
What are the two types of urethritis?
Gonococcal e.g. Gonnorhea o non Gonococcal e.g. Chlamydia or M genitalium or T vaginalis
What are the risk factors for urethritis?
Under 25 year olds, men who have sex with men, STI, recent new sexual partner, more than one partner in the last year
What are the symptoms of urethritis?
dysuria, penile irritation and discharge from urethral meatus, sexual history shows risks, epeidymitis and reactive arthritis
What is reactive arthritis?
sterile inflammatory arthritis caused by distant infection producing an autoimmune response certain joints - usually from chlamydia
What are the differential diagnosis for urethritis?
Balantitis which is inflammation of the glans penis which present with pruritus, erythema, discharge between the foreskin and glans, acute prostatitis and cystitis
What investigations would you preform for urethritis?
Urethral gram stain under microscopy using urethral swabs, first void urine for NAAT, Midstream urine dipstick, triple site testing for culture, further STI screening
What is the management for Urethritis?
Antibiotic management e.g. if gonococcal will give ceftriaxone and azithromycin both single dose, if non-gonococcal will give doxycycline for 7 days or azithromycin single dose
What is the long term management for urethritis?
abstain from sex for 7 days, test of cure is one, condom use and sex safe
Other than oligoarthritis in the lower imp joints, What are the other symptoms of reactive arthritis?
conjunctivitis, urethritis, cervicits, prostatitis, ulcers, malaise and fatigue,
What is the treatment for reactive arthritis?
treated with NSAIDs or local steroids
